
ADDITIONAL INFORMATION REGARDING CENTURYLINK™ PRODUCTS AND SERVICES FOR BUSINESS CUSTOMERS
GENERAL:
GENERAL: All products and services are available for business customers only. Services and offers are not available in all areas. All products and services are governed by tariffs, terms of service, or terms and conditions accessible through www.centurytel.com/Pages/AboutUs/Legal (“Website”), incorporated here, and provided to you by the CenturyLink operating company serving your location. CenturyLink may change or cancel services and rates, or substitute similar services at its sole discretion without notice. All services require credit approval and may require a deposit. Additional restrictions apply.
TAXES, FEES, AND SURCHARGES: Taxes, fees, and surcharges apply to all services, and include federal and state-mandated and permitted charges, cost recovery charges, state and local fees that vary by area, and certain in-state surcharges. Cost recovery fees are not taxes or government-required charges use. Taxes, fees, and surcharges apply based on standard monthly, not promotional, rates.
BUNDLE MONTHLY RATES: Monthly rates for bundle services apply while customer subscribes to all qualifying services in that bundle. If one or more services are cancelled, the standard monthly fee will apply to each remaining service(s).
LOCAL SERVICES: Local phone services include direct-dial local calling, a number of calling features determined by the selected local service plan, and choice of CenturyLink long distance plans (additional charges apply).
LONG DISTANCE SERVICES: Monthly recurring charge applies to one business phone line with direct-dial, nationwide voice calling; excludes commercial use, data and facsimile services (including dial-up internet connections, data services, and facsimile (each billed at $0.10/minute)), conference lines, directory and operator assistance, chat lines, pay-per-call, calling card use, or multi-housing units. International calling billed separately at rates listed at Website. For Block of Time services, block minutes do not apply to operator services, switched data services or international calls, usage in excess of block minutes generates additional, per minute charges to customers, and unused block minutes do not carry over to next month.
BUSINESS-CLASS HIGH-SPEED INTERNET (HSI): Qualifying local phone service plan and a two-year term agreement are required. An additional monthly fee (including professional installation, if applicable) and a separate shipping chatge will apply to customer’s modem or router. An activation fee also applies. An early termination fee applies if customer terminates Business-Class HSI before the end of the applicable term agreement ($199 for former EMBARQ territories, and the applicable monthly recurring service charge multiplied by the number of months remaining in the minimum service period, up to $200, for former CenturyTel territories). Performance will vary due to conditions outside of network control and no speed is guaranteed. Consistent speed claim as well as claim that your connection to CenturyLink’s network is 100% yours is based on CenturyLink providing Business-Class HSI subscribers with a dedicated virtual circuit connection to the CenturyLink central office. $150 wireless router or $99 wired router charge applies if customer terminates services and fails to return router within 30 days.

Suspected misuse or abuse of any computer support may result in termination.
