
ADDITIONAL INFORMATION REGARDING CENTURYLINK™ PRODUCTS AND SERVICES FOR RESIDENTIAL CUSTOMERS
GENERAL: All products and services are available for residential customers only. Services and offers are not available everywhere. All products and services are governed by tariffs, terms of service, or terms and conditions accessible through www.embarq.com/ratesandconditions ("Website"), incorporated here, and provided to you by the CenturyLink operating company serving your location. CenturyLink may change or cancel services and rates, or substitute similar services at its sole discretion without notice. All services require credit approval and may require a deposit. Additional restrictions apply.
TAXES, FEES, AND SURCHARGES: Taxes, fees, and surcharges apply to all services, and include federal and state-mandated and permitted charges, cost recovery charges, state and local fees that vary by area, and certain in-state surcharges. Cost recovery fees are not taxes or government-required charges use. Taxes, fees, and surcharges apply based on standard monthly, not promotional, rates.
BUNDLE MONTHLY RATES: Monthly rates for bundle services apply while customer subscribes to all qualifying services in that bundle. If one or more services are cancelled, the standard monthly fee will apply to each remaining service(s).
LOCAL SERVICES: Local phone services include direct-dial local calling, a number of calling features determined by the selected local service plan, and choice of CenturyLink long distance plans (additional charges apply).
LONG DISTANCE SERVICES: Monthly recurring charge applies to one residential phone line with direct-dial, nationwide voice calling; excludes commercial use, data and facsimile services (including dial-up internet connections, data services, and facsimile (each billed at $0.10/minute)), conference lines, directory and operator assistance, chat lines, pay-per-call, calling card use, or multi-housing units. International calling billed separately at rates listed at Website. For Block of Time services, block minutes do not apply to operator services, switched data services or international calls, usage in excess of block minutes generates additional, per minute charges to customers, and unused block minutes do not carry over to next month. Customers must choose either Embarq Communications, Inc., if in former EMBARQ territories, or CenturyTel Long Distance, LLC, if in former CenturyTel territories, as your IntraLATA and InterLATA toll carrier
HIGH-SPEED INTERNET (HSI): Qualifying local phone service plan and a one or two-year term agreement are required (after which the rate reverts to the then-current standard rate). An additional monthly fee (including professional installation, if applicable) and a separate shipping charge will apply to customer’s modem or router. A one-time, High-Speed Internet activation fee also applies. A $99 early termination fee applies if customer terminates HSI before the end of the applicable term agreement. Performance will vary due to conditions outside of network control and no speed is guaranteed. Consistent speed claim as well as claim that your connection to CenturyLink’s network is 100% yours is based on CenturyLink providing HSI subscribers with a dedicated virtual circuit connection to the CenturyLink central office. $150 wireless router or $99 wired router charge applies if customer terminates services and fails to return router within 30 days.
RESCUEITSM: All support services: Microsoft Windows® 2000 or newer operating systems only. Suspected misuse or abuse of any computer support may result in termination. CenturyLink reserves the right to limit the amount of time spent on any single issue and recommend the dispatch of an in-home technician, at additional costs to the customer, in order to resolve the issue, and may decline certain services or operations outside of the scope of the service. Protection Plan: Requires one-year term commitment. A $50 early termination fee will apply if customer terminates service during the term commitment. One personal computer only. In-Home Computer Support: Available for installation, reinstallation or system recovery only for Microsoft Windows® 2000 or newer operating systems only. All necessary connections require customer-supplied wiring and adaptors.
DISH NETWORK® SATELLITE TV
CREDIT FOR 12 MONTHS
Requires 24-month commitment to Digital Home Advantage. A customer who subscribes to qualifying programming will receive a credit for each of the first 12 months as follows: $15 per month with America’s Top 120, America’s Top 120 Plus, America’s Top 200 or America’s Top 250; $10 per month with DishLATINO Clásico, DishLATINO Dos or DishLATINO Max. Customer must maintain qualifying programming to receive all credits. Customer will forfeit credit in the case of a downgrade from qualifying programming or service disconnection prior to 12 months of credits. Maximum number of months with credits is 12.
3 FREE MONTHS OF HBO AND SHOWTIME PACKAGES
Requires 24-month commitment to Digital Home Advantage. A customer who subscribes to qualifying programming (a minimum of America’s Top 120, DishLATINO Clásico or a qualifying International package) plus qualifying premium movie packages will receive a credit equal to the monthly cost of the movie packages for each of the first 3 months. After 3 months, movie package programming will continue to be billed at then-current price unless customer elects to downgrade.
If customer downgrades within 30 days of end of 3-month period there will not be a Change of Service Fee for this downgrade.
DIGITAL HOME ADVANTAGE
ALL EQUIPMENT REMAINS THE PROPERTY OF DISH NETWORK AT ALL TIMES AND MUST BE RETURNED TO DISH NETWORK WITHIN THIRTY DAYS OF ACCOUNT DEACTIVATION OR CUSTOMER MUST PAY AN UNRETURNED EQUIPMENT FEE RANGING FROM $100 TO $400 PER RECEIVER. Customer pays a $99 Activation Fee. The $99 Activation Fee will be waived for a customer who agrees to a 24-month commitment. If qualifying service is terminated prior to end of 24 months, a cancellation fee equal to the lesser of $420 or $17.50 per month for each cancelled month of service will apply. Customer will be required to pay a lease upgrade for select receivers, based on model and number of receivers. The lease upgrade fees are not deposits and are non-refundable. Offer requires qualifying programming purchase. Customer must provide Social Security Number (or the last 4 digits of the Social Security Number) and a valid major credit card. Participation is subject to credit approval. Customer will be charged a monthly equipment rental fee for each receiver beyond the first, based on model of receiver. Participating retailers may require additional terms and conditions. The first DISH Network bill covers the first two months of service. All future bills will be for one month of service. With 24-month commitment, customer will receive the Service Plan at no cost for the first 6 months. At the end of the 6-month period, customer must call DISH Network to cancel Service Plan or a $6.00 monthly charge will apply for the service. There will not be a Change of Service Fee for this downgrade.
Digital Home Advantage offer is available from DISH Network and participating retailers for new, first-time DISH Network residential customers (only 1 per household) in the continental United States through 5/31/10. All prices, packages and programming subject to change without notice. Number of channels may decline. Local and state sales taxes may apply. Where applicable, monthly equipment rental fees and programming are taxed separately. Activation Fee is completely non-refundable once the system is activated. The Digital Home Advantage includes a satellite TV system with an eligible configuration of up to 6 tuners, Standard Professional Installation and choice of qualifying programming. For this offer, Standard Professional Installation includes typical installation of one single-dish antenna configuration, typical hook-up of up to 6 tuners to up to 6 TVs and equipment testing. More complex installations may require additional fees; other installation restrictions apply. Prices valid at time of activation only; additional fees will apply to upgrade after installation. No more than 6 leased tuners per account are permitted. No more than 3 leased receivers per account are permitted. Any unreturned equipment fees will automatically be charged to the customer’s DISH Network account or the customer’s credit or debit card provided to DISH Network. DISH Network shall determine eligibility for this offer in its sole and absolute discretion.
